Process Overview

These instructions will assist you in downloading the necessary forms to be submitted to DENSPRI for account activation. Forms can be sent by fax, standard mail or overnight delivery to activate an account.

There are four steps to the compliance process:

  • Provide information about your company
  • Read and sign the service agreement and permissible uses documents indicating your legal use of the data obtained.
  • Fill out and sign the appropriate state(s) agreement forms
  • Once the application documents are signed,
